For decades, inflammation has been proposed as a key promoter for atherosclerosis and its complications. However, studies on the use of drugs to target the excess inflammation in CVD are limited. In 2017, the Canakinumab Anti-inflammatory Thrombosis Outcome Study (CANTOS) trial confirmed the key role of inflammation on atherosclerotic disease. Canakinumab is a monoclonal antibody that blocks an inflammatory pathway mediated by IL-1β. The results of the CANTOS trial opened a new era of investigating new therapeutics targeting inflammation for CVD secondary prevention. This review presents the canakinumab's pharmacology, current clinical development status and regulatory perspectives.Aim Determine if incentive spirometry (IS) values correlate with postoperative pain control. Design Prospective observational study. Setting & participants A total of 100 patients undergoing major abdominal procedures at the University of North Carolina Medical Center. Interventions Patients studied as a single cohort. All patients received thoracic epidural analgesia preoperatively. Outcome Preoperative and daily postoperative numeric pain scores, subjective pain description and IS values were collected for all patients. Results There was a strong correlation with IS values relative to baseline for both the numeric pain scores (p less then 0.0001), postoperative day (p less then 0.0001) and the subjective pain score (p less then 0.0007). Conclusion IS values are an objective surrogate data point for pain control after surgery, particularly when followed over time and compared with a preoperative baseline value.Although there has been a drive toward pain self-management, self-management has not yet in my opinion been successfully defined or evaluated and neither has it been consistently translated from idea to practice. In this perspective article, I identify gaps in our approach to pain management and argue that even though we know life context plays a huge role in influencing our health, by and large we fail to take this into account. I argue for a shift in focus away from the 'self' and explore how we might be able to do this within the constraints of our tired and over-stretched health system. This study explored difficulties in meal preparation experienced by adults with moderate to severe acquired brain injury (ABI) and available compensatory strategies from both ABI individuals' and caregivers' perspectives. Further, this study investigated their opinions on potential benefits, barriers and facilitators to the use of the Cognitive Orthosis for coOKing (COOK) in their living environment. Using a qualitative descriptive approach, semi-structured individual interviews and focus groups were carried out with adults with moderate to severe ABI (  = 20) and formal and informal caregivers (  = 13) in Ontario and Quebec, Canada. A qualitative analysis based on Miles etal.'s approach was used. According to participants, cognitive, physical, psychosocial dysfunctions and lack of availability of supportive caregivers were the main difficulties that impede persons with ABI from engaging effectively in meal preparation tasks.
