Skin tags and moles are among the most-asked-about minor procedures in Birmingham. Both seem simple, yet assessment and removal differ in important ways.A skin tag (acrochordon) is a soft, benign skin growth usually on the neck, armpits, eyelids or under the breasts. It is almost never cancerous.A mole — melanocytic naevus — is a cluster of pigment cells. While the vast majority are benign, melanoma can develop in moles, so every mole we remove goes for pathology. This single difference — histology — separates a doctor-led mole removal from a beauty-clinic "mole removal".Skin tag removal techniques include cryotherapy with liquid nitrogen for small surface tags (1–2 sessions, may darken first), hyfrecation electrocautery for small to medium tags (single session, scab heals 5–7 days), and surgical snip with cautery for larger tags or those on a stalk (single session, immediate result). All quick — under half an hour for several lesions. Local anaesthetic is needed for the snip technique.Moles should be removed by excision with sutures, shave excision, or punch biopsy — all routes that allow histology. All three allow histological diagnosis.We do not laser or freeze moles because that destroys the lesion before pathology. If forehead filler turns out to be melanoma after laser, the diagnosis can be missed entirely. Demand excision plus histology for moles.DPN — dermatosis papulosa nigra — is benign and presents as dark facial papules in skin of colour.
