Background Tracheo-innominate artery fistula (TIF) is a life-threatening complication of tracheostomy. We describe perioperative management for innominate artery transection in a case with TIF. Case presentation A 4-year-old Japanese female with Marshal-Smith syndrome presented for management of TIF. She underwent tracheostomy at the age of 3 months and an uncuffed tracheostomy tube was inserted. One month before admission to our hospital, intermittent tracheal bleeding, suggesting TIF, occurred. Although we considered to change to a cuffed endotracheal tube, craniofacial abnormality suggested difficult oral intubation, and there was a possibility of rebleeding. Finally, innominate artery transection was performed under total intravenous anesthesia without changing the tracheostomy tube. Surgery completed uneventfully and she received mechanical ventilation under sedation for a day, followed by weaning without complications. Conclusions A cuffed tracheostomy tube should have been inserted before surgery for effective hemostasis against sudden bleeding from TIF even though conversion to oral intubation was difficult.Purpose To determine whether rectal displacement devices (RDDs) have a prostate-stabilizing effect during prostate external beam radiotherapy (EBRT). Methods A systematic literature search using the PubMed database from January 1, 2000 to December 30th, 2019 was conducted. The effect of RDDs on inter- and intra-fractional prostate displacements was extracted. Results From 356 articles identified via the PubMed database and hand search, 21 articles were included in the systematic review. There was no randomized study. Twelve studies evaluated the role of the endorectal balloon (ERB) in managing prostate motion. Four studies reported the effect of hydrogel spacer on prostate motion. Four studies examined the effect of the rectal retractor (RR) on intra-fractional prostate motion, and only one study assessed the impact of ProSpare (Nottinghamshire, UK) in reducing prostate motion. Conclusion Using an ERB significantly reduces intra-fractional prostate motion. This prostate-stabilizing effect of the ERB can translate into reduced planning target volume (PTV) margins and additional rectal dose sparing. Even with an ERB in place, inter-fractional prostate displacements are seen. As a consequence, ERB application does not obviate daily verification; however, this is not a crucial topic because pretreatment imaging is always done nowadays. As compared with ERB, the hydrogel spacer significantly reduces rectal dose and toxicity without influencing prostate immobilization. The RR can increase prostate and rectal inter- and intra-fractional stability without a clear influence on the reduction of rectal toxicity. Finally, it is unclear whether ProSpare is a suitable device reducing prostate motion. Further study will be required to clarify whether the prostate-stabilizing effects of the ERB and RR can result in a safe reduction of PTV margins and further sparing of organs at risks, especially the rectum.Recent increases in the availability of pre-exposure prophylaxis (PrEP) internationally have widened the range of HIV prevention strategies available to gay and bisexual men. This paper aimed to quantify the use of different risk reduction strategies and compare characteristics of men who rarely use any strategies with those who regularly use condoms or PrEP. Using national cross-sectional data from the Gay Community Periodic Surveys in Australia, comparisons were made between regular condom users (37.1% of participants), PrEP users (50.1%), and men who infrequently used any risk reduction strategies (12.8%). The proportion of gay and bisexual men reporting infrequent use of HIV risk reduction strategies has decreased in recent years. These men reported less frequent high-risk sexual practices than PrEP users and also reported less recent HIV testing than either PrEP or condom users. Men who infrequently use HIV risk reduction strategies should be encouraged to test for HIV more regularly and to utilise prevention strategies that align with their infrequent high-risk behaviour.Mobile phone technology may help sustain reductions in HIV/STI transmission risk behaviors among female sex workers (FSWs). We hypothesized that FSWs who received brief risk reduction counseling and theory-based safer sex maintenance text messages over a 24-month period would have fewer incident HIV/STIs and report greater maintenance of safer sex practices compared to FSWs who received counseling and texts on maintaining general health. The electrocatalytic activity was investigated with cyclic voltammetry in the range pH 2.0 to pH 11.0, and the best electrocatalytic performance of the electrocatalyst was obtained at pH 10.0. By taking advantage of the electrocatalytic activity of the dimeric quinoid brucine toward HA, we have developed an electrochemical sensor for HA measurements based on a brucine/SWNTS-modified GC electrode using amperometry with the applied potential of + 0.1 V (vs. Ag/AgCl). Under the optimized conditions, the current response toward HA concentration shows a linear relationship in the dynamic ranges of 0.1-10 μM and 10-1000 μM with a detection limit of 0.021 μM based on the 3σ criterion. The sensor was used to assay HA in pharmaceuticals including hydroxyurea tablets and pralidoxime iodide injections with satisfactory results. The spike-and-recovery for samples of tap water (n = 9) and lake water (n = 9) was within 97.17-100.16%.
