A contemporary assessment of their in vitro potency against a global CR-PA collection and an assessment of carbapenemase diversity are warranted. Isolates determined as CR-PA by the submitting site were collected from 2019-2021 (17 centers in 12 countries) during the ERACE-PA Global Surveillance Program. Broth microdilution MICs were assessed per CLSI standards for ceftolozane/tazobactam, ceftazidime/avibactam, ceftazidime, and cefepime. Phenotypic carbapenemase testing was conducted (modified carbapenem inactivation method (mCIM)). mCIM positive isolates underwent genotypic carbapenemase testing using the CarbaR, the CarbaR NxG, or whole genome sequencing. The MIC50/90 was reported as well as percent susceptible (CLSI and EUCAST interpretation). Of the 807 isolates, 265 (33%) tested carbapenemase-positive phenotypically. Of these, 228 (86%) were genotypically positive for a carbapenemase with the most common being VIM followed by GES. In the entire cohort of CR-PA, ceftolozane/tazobactam and ceftazidime/avibactam had MIC50/90 values of 2/ > 64 and 4/64 mg/L, respectively. Saturated buffers are a relatively new conservation practice that diverts tile water into a distribution tile installed in a riparian buffer parallel to a stream with the intent of enhancing NO3-N processing within the buffer. In this study, tile NO3-N concentration reductions were characterized through two different saturated buffers at a working farm site in eastern Iowa. Study objectives were to (1) evaluate the hydrogeology and water quality patterns in the saturated buffer and (2) quantify the reduction in tile NO3-N concentration from the saturated buffer installation. Results showed that the two saturated buffers are reducing NO3-N concentrations in tile drainage water from input concentrations of approximately 15 mg/l to levels  less then  1.5 mg/l at the streamside well locations. The reduction occurs rapidly in the fine-textured and organic-rich alluvial soils with most of the reduction occurring within 1.5 m of the distribution line. Denitrification is hypothesized as being primarily responsible for the concentration reductions based on soil and water chemistry conditions, completion of a geophysical survey (quantifying low potential for N loss to deeper aquifers), and comparisons to other similar Iowa sites. The study provides more assurance to new adopters that this practice can be installed in many areas throughout the Midwestern Cornbelt region. Stratification of patients who undergo curative resection for early gastric cancer (EGC) is warranted due to the heterogeneity in the risk of developing extragastric recurrence (EGR). Therefore, we aimed to stratify the need for postoperative surveillance for EGR detection in patients with EGC by developing a model for predicting EGR-free survival. This retrospective cohort study included patients who underwent postoperative surveillance after curative resection of EGC (n = 4149). Cox proportional hazard models were used to identify predictors to build a model for predicting EGR-free survival. Bootstrap-corrected c-index and calibration plots were used for internal and external (n = 2148) validations. A risk-scoring system was constructed using variables significantly associated with EGR-free survival pathologic T stage (pT1b[sm1], hazard ratio [HR] 4.928; pT1b[sm2], HR 5.235; pT1b[sm3], HR 7.748) and N stage (pN1, HR 4.056; pN2, HR 9.075; pN3, HR 30.659).
