. An 8-year-old 36.3-kg (79.9-lb) spayed female Rottweiler was evaluated because of anorexia and vomiting. Extrahepatic biliary obstruction (EHBO) secondary to pancreatitis was suspected on the basis of results from serum biochemical analyses, CT, and cytologic examination. Only marginal improvement was observed after 24 hours of traditional medical management; therefore, novel continual biliary drainage was achieved with ultrasonographically and fluoroscopically guided placement of a percutaneous transhepatic cholecystostomy drainage (PCD) catheter. Within 24 hours after PCD catheter placement, the dog was eating regularly, had increased intestinal peristaltic sounds on abdominal auscultation, no longer required nasogastric tube feeding, and had decreased serum total bilirubin concentration (7.7 mg/dL, compared with 23.1 mg/dL preoperatively). Bile recycling was performed by administering the drained bile back to the patient through a nasogastric tube. The PCD remained in place for 5 weeks and was succesns, and that additional research is warranted regarding clinical use, safety, and long-term results of this procedure in veterinary patients, particularly those that have transient causes of EHBO, are too unstable to undergo more invasive biliary diversion techniques, or have biliary diseases that could benefit from palliation alone. To investigate job satisfaction and engagement among credentialed veterinary technicians (CVTs) employed in the United States. 873 CVTs who responded to an internet-based survey in 2017. A survey was conducted to collect information on demographics, individual engagement, and job satisfaction among a convenience sample of CVTs in the United States. Only responses from those employed in small animal practice were included. Demographic and job-related factors were evaluated for associations with individual engagement and job satisfaction. The mean (SD) score for overall individual engagement (7-point Likert scale, with 7 representing strong engagement) was 4.9 (1.0) and for job satisfaction (7 representing extreme satisfaction) was 5.4 (1.5). Factors associated with lower individual engagement and lower job satisfaction included most frequently working overnight shifts and having more veterinarians in the respondent's practice, whereas holding a supervisory role, receiving a higher hourly wage, and haviand increased job satisfaction. Employers should regularly review factors identified as negatively associated with job satisfaction and engagement and do their best to mitigate them.Context/Objectives Various clinical application of the five times sit-to-stand test (FTSST), with or without hands, may confound the outcomes to determine the lower limb functions and mobility of individuals with spinal cord injury (SCI). This study assessed the concurrent validity of the FTSST in ambulatory individuals with SCI who completed the test with or without hands as verified using standard measures for lower extremity motor scores (LEMS) and functional mobility necessary for independence and safety of these individuals. Design Cross-sectional study. Setting Tertiary rehabilitation centers and community hospitals. Participants Fifty-six ambulatory individuals with motor incomplete SCI who were able to walk independently with or without a walking device over at least 10 m. Outcome Measures Time to complete the FTSST with or without hands according to individuals' abilities, LEMS, and functional mobility. Results Time to complete the FTSST showed moderate-to-strong correlation with the LEMS scores and all functional mobility tests (ρ = -0.38 to -0.71, P  less then  0.05), but only in those who performed the test without hands. By contrast, data of those who completed the FTSST with hands were significantly correlated only with the ankle muscle strength and the functional mobility measures that allow upper limb contribution in the tests (ρ = -0.40 to 0.52, P  less then  0.05). Conclusion Upper limb involvement could confound the outcomes of the FTSST. The present findings suggest the use of FTSST without hands to determine the lower limb functions of ambulatory individuals with SCI.Background Despite the wide use of gadolinium-based contrast agents (GBCAs) for enhanced MRI, their neurochemical and behavioral consequences, if any, remain poorly understood. icFSP1 clinical trial Purpose To investigate the effect of repeated exposure to a linear or macrocyclic GBCA on gadolinium retention in the central and peripheral nervous system of rats and to assess the functional implications of such retention on hippocampal neurogenesis and sensory and cognitive processing. Materials and Methods Seventy male Sprague-Dawley rats (4 weeks old) received intraperitoneal injections of gadoterate meglumine (0.6 or 2.5 mmol per kilogram of body weight), gadodiamide (0.6 or 2.5 mmol/kg), or saline daily for 20 days (February 2018-March 2019). The 5-bromo-2'-deoxyuridine injections were administered every 3 days to determine the number of proliferating cells and the number of newly maturing neurons in the hippocampus. However, it is unknown what is altered in muscles of patients with DM1 as a result of this intervention. The increased exercise capacity suggests that decelerated fat infiltration or increased muscle cross-sectional area (CSA) could be involved. Purpose To assess the effect of this activity-stimulating behavioral intervention on the lower extremity muscles of patients with DM1 with longitudinal quantitative muscle MRI. Materials and Methods In this prospective trial, participants with DM1 were randomized to a behavioral intervention (n = 14) or continued regular care (standard care; n = 13); no age-matched pairing was performed. Participants underwent MRI of the lower extremities at baseline and 10-month follow-up (January 2015 to March 2016). Fat fraction (FF), muscle CSA, and muscle water T2 (T2water) as markers for fat infiltration, muscle mass, and alteration in tissue water distribution (edema), respectively, were assessed with a chemical shift-encoded Dixon sequence and multiecho spin-echo sequence.
