A comprehensive analysis of the studies' themes resulted in five focal points for future research: carers' health literacy, community-based care, carer involvement in transitional care plans, effective communication between inpatient and community health professionals, and the unique experiences of carers from diverse cultural backgrounds.Effective communication and positive relationships between caregivers, older adults, health professionals, and health organizations are highlighted as significant factors in the review. Although knowledge and education hold immense value, further research into the systems that facilitate communication among caregivers, older adults, and health practitioners is necessary, along with the enhancement of health literacy for all caregivers, particularly those from varied cultural backgrounds.The review identifies the necessity of quality communication and strong bonds between carers, older adults, medical practitioners, and healthcare organizations.
