We report the case of a 41-year-old male who presented with an enlarging aneurysm neck one year after clipping. The patient underwent an IMAX-MCA bypass followed by endovascular coil occlusion of the aneurysm neck incorporating an MCA branch origin. To our knowledge, this case represents the first documented IMAX-MCA bypass from a European centre. This case demonstrates that for neurosurgeons experienced in EC-IC bypass surgery, IMAX-MCA bypass is feasible and can be performed safely as long as careful attention is paid to anatomical landmarks and vascular anastomosis principles. CTA-based neuronavigation and micro-Doppler are essential intraoperative tools for identifying the IMAX. To highlight the working strategies used by expert manual handlers compared with novice manual handlers, based on recordings of shoulder and upper limb kinematics, electromyography (EMG), and estimated muscle forces during a lifting task. Novice workers involved in assembly, manual handling, and personal assistance tasks are at a higher risk of upper limb musculoskeletal disorders (MSDs). However, few studies have investigated the effect of expertise on upper limb exposure during workplace tasks. Sixteen experts in manual handling and sixteen novices were equipped with 10 electromyographic electrodes to record shoulder muscle activity during a manual handling task consisting of lifting a box (8 or 12 kg), instrumented with three six-axis force sensors, from hip to eye level. Three-dimensional trunk and upper limb kinematics, hand-to-box contact forces, and EMG were recorded. Then, joint contributions, activation levels, and muscle forces were calculated and compared between groups. Sternoclavicular-acromioclavicular joint contributions were higher in experts at the beginning of the movement, and in novices at the end, whereas the opposite was observed for the glenohumeral joint. EMG activation levels were 37% higher for novices but predicted muscle forces were higher in experts. This study highlights significant differences between experts and novices in shoulder kinematics, EMG, and muscle forces; hence, providing effective work guidelines to ensure the development of a safe handling strategy is important. Shoulder kinematics, EMG, and muscle forces could be used as ergonomic tools to identify inappropriate techniques that could increase the prevalence of shoulder injuries.Shoulder kinematics, EMG, and muscle forces could be used as ergonomic tools to identify inappropriate techniques that could increase the prevalence of shoulder injuries.Aim To characterize the epidemiology of group A Streptococcus (GAS) involved in nonpharyngeal infections sparingly addressed in Lebanon. Materials & methods A collection of 63 nonpharyngeal GAS isolates recovered between 2010 and 2019 from northern Lebanon were analyzed through emm typing, virulence gene profiling, FCT typing and antibiotic susceptibility analysis. Results & conclusion A total of 29 emm subtypes was detected, with emm1 being the most dominant. A great intraclonal divergence driven by the loss and gain of superantigens or by the structural variability within the FCT regions was unraveled. The resistance rates for erythromycin and tetracycline were 8 and 20.6%, respectively. The 30-valent vaccine coverage was 76%. This study evidences the complexity of the neglected GAS pathogen in Lebanon. Many pesticides can antagonize the androgen receptor (AR) or inhibit androgen synthesis but their potential to cause reproductive toxicity related to disruption of androgen action during fetal life is difficult to predict. Currently no approaches for using data to anticipate such effects exist. Prioritization schemes that limit unnecessary testing are urgently needed. The aim was to develop a quantitative to extrapolation (QIVIVE) approach for predicting anti-androgenicity arising from gestational exposures and manifesting as a shortened anogenital distance (AGD) in male rats. We built a physiologically based pharmacokinetic (PBK) model to simulate concentrations of chemicals in the fetus resulting from maternal dosing. The predicted fetal levels were compared with analytically determined concentrations, and these were judged against active concentrations for AR antagonism and androgen synthesis suppression. We first evaluated our model by using and anti-androgenic data f shortening) for chemicals with activity and for minimizing unnecessary testing. https//doi.org/10.1289/EHP6774.Our QIVIVE model newly identified fludioxonil, cyprodinil, and dimethomorph as in vivo anti-androgens. With the examples investigated, our approach shows great promise for predicting in vivo anti-androgenicity (i.e., AGD shortening) for chemicals with in vitro activity and for minimizing unnecessary in vivo testing. selleck inhibitor https//doi.org/10.1289/EHP6774.Grit is one of the non-cognitive variables that has received the most attention in recent years given its relationship to and influence in various aspects of life. There are very few reliable, valid instruments to evaluate it in Spanish-speaking countries. Because of that, the aim of this study is the development and validation of a new scale to evaluate grit in Spanish-speaking contexts. We used a sample of 531 Spanish participants (60% women) from the general population (Myears = 38.60, SDyears = 14.90). We examined the structure and measurement invariance of the instrument. We calculated the instrument's reliability and obtained evidence of validity in relation to other variables. We examined the differences in grit as a function of gender and age. The factorial analyses confirmed the unidimensionality of the instrument, along with the measurement invariance of the scores with respect to sex and age. The new grit scale demonstrated excellent reliability (α = .94; ω = .94). We found clear evidence of validity in relation to other variables; the Grit short scale (r = .691), self-control (r = .595), self-efficacy (r = .703), and conscientiousness (r = .661). The new scale for evaluating grit (Oviedo Grit Scale) is essentially unidimensional, and scores produced by it exhibit excellent indicators of reliability and validity.
