Within this study, we scrutinized botanical and toxic synthetic pediculicides, juxtaposed with 4% dimeticone liquid gel, for their efficacy in treating head lice in Thailand. A study evaluated the pediculicidal activity (outside a living organism) of various pediculicidal shampoos found in Thai drugstores, contrasting their results with a 4% dimeticone liquid gel. Lice-killing shampoos were chosen for their active ingredients, which included 1% permethrin, 0.6% carbaryl, 0.15% Stemona root crude extract, or combinations of plant extracts, differing from dimeticone's non-chemical, physical approach to lice control. The application of 4% dimeticone liquid gel, using the prescribed method detailed by the manufacturer, led to the complete elimination of 100% of head lice in just 15 minutes. Conversely, alternative pediculicide products exhibited far less efficacy, resulting in mortality rates of 0% to 500% with a 10-minute treatment, and 170% to 600% mortality following a 30-minute treatment. To further validate the effectiveness of 1% permethrin in treating head lice infestations among schoolchildren, we also implemented a comprehensive clinical evaluation. Despite two applications of 1% permethrin shampoo, not a single one of the 26 children evaluated in this clinical trial achieved a cure. The Thai study's results indicate that a 4% solution of dimeticone showed greater success in eliminating head lice ex vivo, contrasting chemical and botanical pediculicides. However, 1% permethrin displayed minimal efficacy in laboratory and clinical assessments.
